SCHOLARSHIP CRITERIA CONSENT

I have read the scholarship criteria listed above and meet all the requirements.(Required)
I understand that if I am approved, I will need to register for the classes at least 30 days in advance of the class and directly with the association offering the classes and provide them with my scholarship coupon.(Required)
I understand if I am a no-show, do not register the class(es) or do not pass the class exams, I will forfeit the remainder of my scholarship.(Required)
